Before submitting this form perform these steps for HID kits:
Start by performing the following steps to determine which part is faulty:
swap bulbs from side to side and see if the problem follows the bulb, if it does the bulb is bad.
next swap igniters(if equipped) next the ballast, the problem will follow the bad part.
once you determine which part is faulty please complete the form bellow to submit your claim, we will respond in 2 business days or less.
please have your receipt or proof of purchase, without it we will not be able to provide warranty service.
